If you have NEVER been registered with the CDHO then click ‘To Register’ to apply to access the examination and pay the non-refundable $50.00 Application Fee.

After receipt of your application payment you will be able to log into the Self Service Portal with your email and chosen password. The Online Exam can be accessed from the Self Service Portal.
